Ask Well: Fighting Bags Under the Eyes

Q

Is there an effective treatment for dark under-eye circles?

Reader Question • 1282 votes
A
It depends on what caused them, said Dr. Lisa Donofrio, an associate clinical professor of dermatology at the Yale University School of Medicine. Some people have dark “bags” or “allergic shiners” under their eyes because of allergies or congestion, preventing the normal draining of blood from that area. Lack of sleep, of course, can lead to bags, too.
For puffiness, the best thing to do is to treat the allergies or get enough sleep, or cover up the bad days with cosmetics, said Dr. Michael Reilly, a facial, plastic and reconstructive surgeon at Georgetown University Medical Center.
For darkness under the eyes, he recommends cosmetics. Cold compresses and the tannic acid in cold tea bags might also help constrict the blood vessels under the eyes and lighten the color, he said — an approach that can also work for people who have particularly thin skin under their eyes, allowing reddish-purple muscle to show through. But steroid creams are a bad idea, Dr. Reilly said, because they can thin the skin even further, making dark circles more likely.
Topical treatments like retinoids might also help people with thin skin, Dr. Donofrio said, “but in general this cause is pretty hard to treat.”
In rare cases, the skin under the eyes becomes stained because of natural skin pigment or leaky blood vessels. Topical cosmetic treatments like brightening and vitamin creams might help, she said. But laser treatments that target pigment discoloration are generally more effective, she added.
Bags under your eyes can also be caused by extra fat deposits in the lower eyelid, which can be removed through a surgical procedure called a blepharoplasty.
Too little fat, a common occurrence with aging, can likewise create a trough under the eye. Age-related sagging can be treated with dermal or facial fillers such as RestylaneJuvĂ©derm and Perlane, which typically cost $700 to $800 per treatment, but don’t last more than a year or so, said Dr. Alexes Hazen, director of the aesthetic surgery center at NYU Langone Medical Center. A more permanent solution, costing a few thousand dollars, would be to transplant a tiny amount of fat from the belly to fill the trough, she said.
The important thing for most people is to take good care of the skin to help keep it taut and minimize the bags that can come with aging, said Dr. Scot Glasberg, president of the American Society of Plastic Surgeons and a surgeon at Lenox Hill Hospital in New York City.
“The earlier you do the correct things” to improve general skin tone and appearance — including not smoking, getting adequate sleep, reducing stress and limiting sun damage, he said — “the better it is for the long term.”
